A geriatric assessment is the first step to understanding the care gap and recommending a care plan.
Service Description
During a Geriatric Assessment, We visit with the prospective client at his/her place of residence and we deep dive into the life(medical history, medications, living condition, diet, nutrition, family situation, support system, allergies, physical, social and mental health) of the client/care receiver to get a good understanding of the care needs. Post assessment, we present client with a proposed Personalised Care Plan.
Cancellation Policy
1. Fullcircle Day Centre is an activity based centre. By registering for this event, you confirm that you are medically, physically, mentally and emotionally fit to participate in the day centre activities. Fullcircle Geriatrics will not be liable for any incidents that may occur during the event. 2. Dependent seniors must be accompanied and supported by personal caregiver/family member 3.Pictures and Videos from all our events are used to promote the fullcircle community via our digital platforms. 4. Payment for this service is non-refundable, non- transferrable, must be paid before service and cannot be deferred to a later date 5. Clients can only participate in the specific session they paid for. i.e. Clients who pay for Monday sessions cannot attend Tuesday session or substitute payment for other sessions.
