A geriatric assessment is the first step to understanding the care gap and recommending a care plan.
Service Description
During a Geriatric Assessment, We visit with the prospective client at his/her place of residence and we deep dive into the life(medical history, medications, living condition, diet, nutrition, family situation, support system, allergies, physical, social and mental health) of the client/care receiver to get a good understanding of the care needs. Post assessment, we present client with a proposed Personalised Care Plan.
Cancellation Policy
1. Fullcircle Day Centre is an activity based centre. By registering for this event, you confirm that you are medically, physically, mentally and emotionally fit to participate in the day centre/memory cafe activities. Fullcircle Geriatrics will not be liable for any incidents that may occur during the event. 2. Dependent seniors must be accompanied and supported by a personal caregiver/family member 3. Payment for this service is non-refundable, non- transferrable, must be paid before service and cannot be deferred to a later date 4. Pictures, Videos and data collected at our events will be used for the purpose of growing the fullcircle community.
